Looks like the hole has been milled off center on that piece causing the threads on the grub screw to bite into the delrin and break through the surface. The hole on the outside and the hole on the inside probably don't align correctly when closed.

No that's not possible. We assemble each tool here at CK. It wouldnt be possible to assemble the tool if both parts don't align.
