He had a player writhing in agony/was concerned about him/was dealing with sorting out a replacement/etc... it wouldn't be surprising if he'd assumed there was a yellow even though he didn't see one shown.

surely, for a tactical advantage a manager should know which opposing players are on a booking

Trying to do Ronaldo skills but ultimately runs 15 yards directly into a player and loses the ball, it was pretty 'special'.
