Indeed, all the diamonds I've found are in open caves mainly and not near lava.
I've never found diamond randomly near lava. I've always exposed it by exploring caves and/or shovelling away dirt and gravel in natural systems.
The algorithm doesn't put diamond within proximity to lava, or require it to be buried behind rock.
No, what you mean is you've never seen the lava it's near. If there's diamond, there's a chance there's lava nearby.
Diamond ore is generally considered to be the most valuable and elusive block in the game. It can be found generally in a vein of 4 blocks, and you may often find lava in its proximity and vice versa.
Whenever I've seen diamond and no lava, mining around the diamond within about twenty blocks spherically normally uncovers lava.

Arrows are made from feathers [chicken, zombies], sticks and flint [gravel]. Remember that on average you will get flint from 10% of all the gravel blocks you mine, so hit a few gravel blocks to get a good supply of flint.
