You don't have to we play a lot of vanilla maps as well .

Yes it is included i don't think any game modes are excluded from standard

Personally I mainly play vanilla, the odd silk road from china rising dlc and the recent final stand maps.

^^ pretty much all maps support all game types (I think anyway).

Erm not sure on what map the most honestly it's whatever rotation the server has set.

I play a lot on lockers 64 man i just love the madness, I think mynight you joined me one night last week, I play under [OCuK}Hazy13 on the VIVA lockers 24/7 server.

If not I think these are the ones you get for gaining stars after reaching max rank (When you were stuck at the previous max ranks).
