The visual settings for Ps4 are usually the PC high settings. I am not sure where you are getting this low settings. Find me a game or article where it's stated the PS4 is running low settings. Here is a comparison of the BF1 beta where Ps4 is targeting a 60fps frame rate.
No where near low settings. Ps4 pro with around double the graphics power should be able to push ultra settings. For how weak the hardware inside the Ps4 is i think it does a really good job.
Because there's a lot of deceptive tweaks that happen on consoles besides just graphical fidelity which make them compete but decrease the enjoyment, like more pop-in. And no, most graphically demanding games that are released on consoles are usually at 30fps, like Forza Horizon 3, DXMD (which doesn't even run at 1080p on XO), etc. just check out digital foundry's videos; I'm not gonna run through all of them but anyone interested will see what the majority is like (for AAA).
