I ran a passmark test for my CPU and i got a 7480 out of 7729. Is this bad? and could it be the problem? i did by the i5 used :/

They get 23.00 average. So maybe your PC is acting normally.
As for Fallout 3 I can't find anything on it with the 3570K/7850.
I kinda ignore Passmark, I don't really find it reliable.
It looks fine on the other hand. Only thing I can think of, is maybe Vista really is chopping performance down...
Do you have any other games you can test with?
Also random question, you have the graphics card on the top PCI-E slot right?
Well, I suppose it wouldn't hurt to replace the HDD if it's rattling a lot.
It's very rare for the CPU to be faulty as well.
On the other hand, lauching games does sound like a possible GPU problem. Could try a different set of AMD drivers if you want.
I'm starting to run out of ideas honestly. That Windows error looks worrying as well, but I'm a bit unsure on what it means.

Might as well give this a go, check the Health tab, and post a screenshot of it, then run the Error Scan. This will take a while, but it will help to check if there's any damaged sectors on your drive.
Sorry if this has been mentioned before but have you tried taking one stick of RAM out at a time to check if that could be the problem? Faulty RAM can cause major issues including OS related ones and go un-noticed during RAM tests.

No probs I hope you find the issue out! What RAM do you have btw? I recently had big issues that caused system errors with ADATA 8GB RAM.

That looks pretty worrying if the Relocated sector count is on warning. Sounds like the drive may be dying.
