NHS=Negligent Health Service

I'm on call this week and dreading it. The demand in Paediatrics has skyrocketed and we're so neck deep in worried well it's hard to weed out and look after the sick ones.

Yes records go missing all the time and I'm dealing with one now for the Coroner.
The secretary has clearly tracked them wrong and doing an Audit Trail nobody on her iFit list knows anything about them.
We will get fined by the Coroner.
Some of the main offenders are Clinicians, the Secretaries track them in for their Clinicians and that's where the trail goes dead because they are supposed to give them back to the Secretary.
Quite often they may give them to a fellow Clinician who then gives them to another with no tracking taking place.
How do you lose an iFit record? that's not supposed to be possible O.o

Surely you can just ignore the last manual scan if you think it was a mistake and look up where it was last auto scanned and it must still be in that zone, it shouldn't take too long to find. Or am I missing something?

Please don't tell me your trust invested in an RFID based record tracking system but then didn't buy/install the RFID sensors in the hospital? xD

The other thing about this - it surely can’t be standard practice to send a text saying you have aggressive cancer? Wouldn’t they at least call you if not ask you to come in to discuss it?

Love the casual “thanks” at the end too.
 
It wouldn’t be normal to send a text for sure. Reading the message this was intended as a task to a GP to do this form for a patient, but someone sent it to all by mistake

Even then tbh its a pretty chappy message. They're asking someone to do something for them at least be polite ffs! On top of that surely such requests should be sent via something like email?
 
I agree. It’s a cockup for sure. An email or an internal ‘task’ message from within the electronic patient record software should have been used, but there is a way that other services in the community can send messages within an SMS system and I expect someone thought they were forwarding it and messed up. It certainly was never intended for a patient looking at what the message was saying
 
A friend's husband, having a first name and surname in the top 10 in the UK, had a letter addressed to him from the hospital. It's a report of a consultation. He read it and thought this isn't me. It was for a man with the same name under the same consultant as he sees. He knew it wasn't for him as the DOB was different, by 7 years.. It's impossible to contact the hospital dept about this as the other man may received my friend's husband's letter.

Don't think this is an isolated case with people with very common names.

why would it be impossible to contact the hospital about it? That is extremely serious and needs investigation. It is the NHS / hospital number that is the primary identifier for patients, regardless of them having the same name and same consultant this should never happen.

There are some depts which are impossible to contact as phone never is answered or engaged. I actually went to the hospital dept to change an appointment as it was during a foreign holiday.

Now I have the phone numbers for the consultants’ secretaries now and appts are changed within a day

We had a bit of a worry this afternoon and I wanted to post what a great experience we had with the NHS.

My daughter (3) was playing on our bed and fell over (just fell over, not off the bed) she cried and it looked like she just landed awkwardly and perhaps winded herself, but while my wife gave her a cuddle about 10s later, she went floppy and her eyes rolled back and wasn't responsive for 3-4 secs. Then she came to and was absolutely fine as if nothing happened. We phoned 111 and they told us to go to A+E just in case (she has had heart issues in the past). My wife and daughter were in and out in just over an hour and they did loads of tests and were thoroughly helpful and informative. They even had a bunch of Christmas gifts for the children that they were handing out.

We got the all clear thankfully, and we're both very happy with our experience.

Big thanks to anyone working for the NHS in these tough times, especially those at St Richards in Chichester :)
 
When I was in hospital recently with the covid, it was a mixed bag experience.

I had to go in via ambulance through A&E. Because I had low oxygen breathing problems I went straight in. But after they did all the standard tests, those ecg heart tests, and drawing blood, I was left on a trolley in A&E for 12 hours. What made it worse was the nurse call button wasn't given to me. I also was given nothing to eat until I managed to attract the attention of a passing staff member.

Then after the 12 hours I was told I was being transported out to an overflow area, which was like a ward with lots of closed door small rooms. I was put in there, and this time was given the nurse call button, and the door was closed. When I needed some help I pressed the nurse call button only to find out later it didnt work! I had to literally shout through the door to get a nurses attention. That whole area was very busy with both staff and some patients walking around. I was in that place for another 10 hours. Though at least they give me something to eat and drink.

Then finally after a total of 22 hours I got a bed on a ward. The nurses and staff there were excellent and all was good.

I suspect the hospital staff were over worked as on multiple occasions I was asking them a question only for them to say they would find out and never came back. Apart from that the staff were friendly. It felt like the hospital was over loaded. I noticed when I was coming off the ambulance there were 9 other ambulances waiting outside A&E.

Sorry, only just had the notification for this.

iFit is our software for tracking hand held patient notes, they have a barcode on them but people don't track them.
Yes you can go down a tracking list but unlike our last system called FileFast, iFit only shows the odd tracking here and there.
I have asked the Health Records Manager if she has a higher access to see all tracking but she sees exactly what I see therefore I'm looking at iPortal which shows a patients journey and contacting everybody who may have had the records.
When you eventually locate them the reply is usually "I don't have time to track them" :(

And then there are those who track the wrong thing:
It could be IPT Notes vol 2 but they track it as Hospital Notes vol 1, oh yes this happens all the time.
The only sure way would be to buy everybody who deals with notes a bar scanner but they are as rare as rocking horse poo and then you'll get them who will say they haven't got time to scan anyway.
