I'd forgotten why I stopped playing this so I remembered my password and long-lost email address I had signed up with and downloaded it again, straight away I couldn't believe that there were still no Mouse-Thumb buttons mappable!
And then I started playing and within 15mins I remembered why I stopped... Sea/Motion-sickness from the stupid (Un-turn-off-able) head-bob, I think it exaggerates the problem for me by being so detailed and having to concentrate so hard looking for clues and movement. It was so bad I couldn't face looking at my monitor for the rest of the night without feeling queasy. Ironically I go off-shore Sea-Fishing regularly and in sometimes very rough seas and have never once been sea-sick. Only time it happens is in certain Games with head-bob and usually you can either turn it off in options or tweak a .cfg to stop it

I have no idea why it gets me so bad, I put it down to the concentration/eye-strain and the weird thing is, it's not even an actual 'Head-bob' it's more subtle than that although I'm sure the required effect is a 'Head-bob' if that makes sense, whatever it is it makes me very, very nauseous very, very quickly. I can't watch 3D Movies for the same reasons although they usually take a little longer and give me more of a splitting headache than nausea.
