Where is the first floor?

Because it is the first floor you reach when entering the house/building. Really the Americans have a much more logical numbers system here.

You're still at ground level, thus ground floor. That's as logical as it gets.

Ground floor can be synonymous with 1st floor, and in America that is often the case. But in the UK the ground floor would have to be the zeroth floor, which is just awkward and wrong because it is the first floor of the building.

If a building has 10 floor and you had to number them you would number them from 1 to 10, when you count on your fingers you count from 1 to 10 and not 0 to 9, when you get £4 change the person behind the toll counts out 1,2,3,£4 and not 0, 1, 2, £3.
 
I see it as ground floor, 1st floor etc.

Because when I go up a floor, I go up a single floor.

So when I am on the ground floor, after I gone up a single* floor, I arrive at the 1st floor. I do not go up a single floor and magically arrived at the 2nd floor.

* note the use of the word single, meaning one, not two.

Do they start with 1 or 0?

They start 1, therefore when you walk into a building you are on the 1st floor

or....walk into a shop and ask for three mars bars. Does the assistant hand over 3 or 4 mars bars?

Walk in to a shop and see a stack of 3 mars bars. Ask for all the mars bars. How many do you get?

Because the ground isn't a mars bar.

People omit 0 because it is often of no consequence. But when I count on my hands I do actually put all my thumbs and fingers down, I start from 0, I just don't say it.

Likewise, the shop assistant is counting from 0. You have to have 0 to begin with. Unless you are stealing. Don't steal the ground floor!

That's ridiculous. That's implying that every thing has an extra thing because it's zero.
 
Someone has already mentioned it but its comparable to a staircase / ladder. When you step onto a ladder / staircase, you are elevated by one rise, you are therefore on the first rung/step. You wouldn't say you were on the first rung of a ladder whilst you were stood next to it.

I believe this is something significant in the history of mathematics, can't remember if it was India or the middle east that came up with the idea of zero.

The first rung on the ladder is not on the ground is it.

The ground is on the ground. Ergo the ground floor is on the ground floor.
