You look bigger without a doubt, but as already said, the angle and the slightly out of focus shot make it very difficult to point to specific areas, I would guess that your upper arm is bigger, but without a like for like shot a guess is all it would be. The only area I would say for sure is bigger regardless of angle is your traps.
That said, if you'd simply posted both pics and asked which was more recent, I'm certain I'd have gone with the second, so you must be doing something right.

Your doctor is a moron. Can you feel a hard lump, be it pea sized or grape sized under your nipple? Not immediately under, it will be quite deep, around the base of your pec.
