The Gallery link, they're 4 sets of UCCW uzip widgets, so you can edit the text and images, backgrounds of the tiles and stuff in UCCW.

Go into Play Store then My Apps and then All, should show what you have had installed at some point

I'm guessing it is accuweather that comes as a standard widget on all Samsung phones.

Not sure what the widget is like from the app on the play store though.
