I really wish people would post what icons, docks, widgets and wallpapers they have used in this thread 
Here's my latest:
Galaxy S3 with Apex.
The clock was made with Zooper
The dock are all Power Toggles 1x1 widgets.
The widgets are all Power Toggles as well using these icons.
I cant remember where I get the wallpaper from, I've had it for years.

It's changed a bit now. It was simple, I use the following:
Nova launcher prime
Eye in the sky weather (for forecast)
UCCW (Time, Temp and 2 bottom buttons which are uccw-playbars from playstore)
Zooper widgets pro (for battery circle, easy to make)
White icons are Holium five.
Simple RSS widget for news stream.
I originally used Zooper to make the white boxes but ended up just adding them to the background in photoshop to have less widgets running.
I use Pie controls on Paranoid Android rom so no navigation bar or status bar

Something a bit different, don't know about the wallpaper though...... so hard to find a nice minimalistic/patterned/textured wallpaper that I really like, always just end up back on the same one within a day

How did you amend the colours of the labels?
This is how mine looks now work in progress I'd say 
Just make sure UCCW is not in hotspot mode, click on the icon and adjust colour from within UCCW, hope this helps.

I'm running Apex Pro, so don't need icons or folders on my screen as I have the App Drawer organised and 'Swipe Gestures' for my mostly used apps (Swipe Up - Phone, Swipe Down - Text etc..)
Thanks, having one problem. The widget on my lock screen stays on small when added but if I swipe to the other lock screen and go back the widget changes to full size. If I expand it to full size and lock then unlock - the widget goes back to small.
