The theme is a generic Desire theme for Cyanogen 6. Power switches are Switch Pro. I've got a CPU usage indicator top left (SeePU).

Here are the dock, icons and wallpaper. You need Launcher Pro to use the dock and icons like I have.

For the people who are using SwitchPro...

Regarding the Data Connection enable/disable (Mobile Network) does the 'Mobile Network' have to be On in order for the switch to work like 'Extended Controls'? Or does it actually turn on and off the mobile network unlike Extended Controls, so that when you hold the power button on top of the phone and you get the phone options it tells you that the mobile network is on or off unlike with extended controls where the mobile network remains constantly on.
