mine was stopping at a red light on an incline.. only for my bike to roll back a little and all my weight switched to the clipped in side and bam I slammed into the floor, actually hurt my leg pretty bad :o pedal went into the bone big time and hurt for a month or so.
 
Didn't have any issues when I first started with clipless, but had a fairly comedy looking incident when I went out with the kids not long after.

Travelling down a quiet road and one of the kids just randomly decides to stop - his brother realises this and manages to stop, but me not paying as much attention as I maybe should have, struggled to stop without crashing into either of them.
Unfortunately, having then come to an expected stop, I forgot I was still clipped in, and it became one of those slow motion "noooooooo" moments, as I then tipped over and fell into the grass at the side of the road, with both kids laughing :)

Absolutely lovely weather down here, but it was windy in some places! I thought I took a wrong turn on my route, but it turns out I didn't. Had to double back on myself a bit.

I need to sort out my bottles and cages: My cages are a total pain to get the bottles out of when riding. I need 2 hands when stopped to get them out :( Also, my bottles have covers on them because there is a lot of dirt and horse poop on the roads I cycle on. Makes them a bit harder to drink from whilst riding.
 
End of the drive at work after an unplanned stop (van appeared from nowhere and I'd not unclipped as hadn't totally stopped).

Middle of a housing estate when fiddling with mudguards, too much looking behind and not enough thinking. Car full of kids & mum stopped to ask if I was ok. Was laughing at myself too much to not be!

In traffic in the pouring rain, clipped in to move and car infront randomly slammed his brakes on for no reason. 4x4 luckily behind had left lots of room and was stationary. Bad language followed.

Last saturday on a group ride, walked the end of a 25% climb. From a flat driveway got some momentum, first downstroke with right leg clipped in ok, failed to clip in left foot, leg flies out sideways (somehow) rather than downwards, compensate and overbalance right side where I'm still clipped in. Didn't quite hit the floor - as at 0 mph I got my right hand down in time! Like a Ninja! :D ;)
Still have a huge bruise on my rump from my saddle nose :o

Unclipped my left foot approaching a junction then decided to try to place my right foot down. :o

Wouldn't have been too bad apart from the local cycling club came around the corner as I was doing my dying fly impression :D
 
Luckily haven't so far but I spent most of my youth riding a road bike with toe clips, so am used to thinking about removing my foot from a pedal before stopping.
 
Forgot about the one going up Britwell Hill in the Chilterns, was knackered and thought id see if i could zig zag it. Anyway, i couldnt! ended up falling in the road! a car parked half way up was having a chuckle :D
 
I've been spanking the Watt Bike in the gym at work during lunch. Sore legs :(

I find my ability to pedal smoothly through the entire revolution goes out of the window when I'm tired as well. Unsurprising really.

Souplesse is massively overrated (in my opinion). I find I'm actually more inclined to pull up on the pedals when I'm tired or working at low intensity and it usually leads to more hamstring soreness then when I'm (relatively speaking) mashing.
 
I remember my first SPD related dismount - overbalanced on the leg I still had clipped in. Thankfully I'd stopped in the middle of nowhere, so no witnesses. Ended up having to take my shoes off to get back off, must looked like a fish flapping around on dry land :D
 
I better get some achievements for tonight's ride home... I absolutely smashed it, was dripping sweat by the time I got home. I managed the 11 or so miles in pretty much bang on 40 minutes, so should average 17mph. I was spinning like crazy - 24mph on 42*17 means cadence of over 120. I dropped the same guy 3 times as I kept getting stopped at the lights, but the acceleration off the line on that bike is pretty impressive so I made short work of him :D
 
Woot, new saddle - Madison Flux, fundamentally identical to the Charge Spoon - came in the post so I've swapped off the rubbish stock one on my single speed commuter. I've given it a ride up and down the street and it's soooooo much more comfortable! I've got the fit all right around the pedals and saddle, but I can't get the reach as aggressive as on my main bike - I'd need a 14cm stem to get the same reach and that's just stupid, particularly when it's a commuter so the more upright position is eminently sensible. I have also dropped the stem a bit, so it's not quite so Dutch.

I might pick up another Flux for my Sunday best bike. Much as I like how neat and tidy the Charge Scoop is, it's just not as comfortable for me.

Sounds like the time one of my SPD cleat bolts came loose and ejected itself somewhere... I felt something was up so unclipped the right and came to a stop... then I realised I couldn't unclip the left at any cost. I ended up slowly lying down on my right then taking my shoe off on the left still stuck to the pedal. I had no idea how to get it off as the cleat was spinning independently of the shoe. After about 15 mins I took a cleat bolt out of the other shoe and managed to screw it in from the bottom of the shoe and snap it out. Nightmare of a time trying to fix it!

Ouch. I've done that too but when i fell over the cleat popped out. It took 4 falls before i realised what was going on and why i kept toppling over :(
(I was doing a 24 hour race and quite tired at the time though)
