You don't really need any. It's a great game no matter how you play it. And as long as you aren't on Death March difficulty it's not over challenging for a first timer.

I'm 63 hours into the main game and in skellige. Reckon i must be getting near the end now

Get used to using Quen a lot.
Make good use of Yrden - the alternate version is handy as well.
I enjoyed using Aard late game - very good for crowds.
As said before, dodge is your friend. Don't try to go toe-to-toe.
Make use of the bestiary and oils if fights are a bit tough.
Take your time, explore and enjoy the beautiful sights.
I played on Blood & Broken bones but I'd probably do Death March if I were to play again. I'd have started playing Gwent a lot earlier too as I avoided it at first.
I haven't bothered playing Gwent since the first tavern at the beginning of the game. I'm on level 9 now. What is the advantage of playing Gwent aside from experience points?

Make sure to disable point of interest markers on the map after you leave White Orchard, keep it enabled there and explore them all. Enable alternative movement response (both in menu). Oh and disable fish eye effect on witcher sense if it bothers you. Many people complain about it and do not even check the menu to see it can be disabled.
Take your time, do not rush, read journal and bestiaries, use full scope of tools in combat (signs, bombs, dodge, roll, riposte..), select proper difficulty (I recommend Death March od Blood and Broken Bones) and graphical settings that will get you 60fps (hardest on framerate is foliage distance, shadow quality and Hairworks).
Then there are couple mods that improve the experience and I consider them must haves: Immersive Cam with its submods (immersive motion, alternative horse control), Immersive HUD with a tweak, Sensible map borders.
